v First Select Your State from drop down Menu from where
you belong.
v After Select, Your State Then
Click on "Apply Online" on the left side of the page and
select and click on "Services on Driving Licence".
v One page will appear with List
of DL services then click on Continue Button for Change Name
Request.
v Enter DL (Driving Licence)
number and Date of birth and click on Get DL Details Button.
v Applicant Name with other
details will display on this page. Select “Yes” from Confirmed that the above
Driving Licence details are mine Menu and
Select Apply to state and RTO and click on Proceed.
v Details of the Driving Licence
page is display and again fill up personal details and address rows. Now click
on Confirm Button to proceed.
v DL services page will display,
select the CHANGE OF NAME IN DL required service checkbox and
click on Proceed.
v Change of Name in DL transaction page is
displayed. Select the required details from drop boxes displayed and click on
Confirm, then Data Accepted Successfully message is displayed.
v To the left side of the page
declaration box and captcha is displayed, Check(Tick) the declaration box and
enter the captcha code then click on the Submit button.
v Acknowledgment form is
generated with the Applicant details and the service requested. (Change of
Name in DL) Click on Next to proceed
v Next Process Click on the radio
button “Upload Documents” to upload the required documents and click on the
“Next” button. like Name Prof
v In the step click on the radio
button “Upload Photo and Signature” for the Applicant to upload the
Photo & signature and click the “Next” button.
v Select the “Fee Payment” and
click on “Next” to continue for fee payment.
